You got the C D Em right for the verses and the C D G Em for the chorus. For bass it is:
(verse)
e:
B:
G:2 fret then third
D:5 fret/or open G string then second
A:3 fret play first
E:

(chorus)
e:
B:
G:2 fret last
D:5 fret/or open G string second
A:3 fret play first
E:3 fret third
