The first bit is right (Bb Gm C/E F).
The D/A should be a D7 chord, and the A/C# A should be Asus4 A.
To save confusion for others here is the whole passage agin but with corrections.

| lostboybinns - 2007-01-10 | Ok here goes...Ive made it as simple as I really can ;-) i.e few bends as possible and sounds to me like he uses his tremolo for the 3rd note but Ive made it easier and replaced with a pull off and hit on :-) oh, and Ive tabbed it so its totally on the B string
sl = slide
(b) = bend the string
h = hit on
p = pull off
