Okay guys you are all very close. Here it is.

Capo 4th and Hold G2 Pattern through out the Verses and Chorus, it's just a simple bass walk up.
Vs & Chorus: G2 G2/A G2/B G2/C
Bridge: D/F# C G2/B C

You can hear the G2 pattern if you listen to the finger picking towards the end of the song, right before the chorus crescendo.

I don't really care for the intro so you are on your own, from what I can gather it is Em/B not just Em.

One more correction, listen to the last line of the chorus. Instead of just playing the bass note walk up they end with the full chords, G2 AM G/B C2. This is only on the "You're worthy to be praised forever and a day" part. Also on the into it's a secondary bass walk up, E is constant play the Em pattern.

Guitar: Em Em/C# D Em/C# on the last run through before the vs. end on D
