Try using a Bb (113311) instead of the C in the second line of the chorus...

Guitar:
There is no A2. It's an Am7 in the intro (regular Am in the rest of the song). The C in the intro is a C2. You can't miss it. During the C chord, the high D (2nd) is the loudest note, and the change from that D to a C is obvious since it's the loudest. ;) With distortion, you shouldn't be using anything except roots and 5th in your chords (power chords). Anything else makes the distortion sound extremely sloppy. I hear lots of people play that way anyway, and it just doesn't sound good.

Bass:
It sounds like this is either a 5 string bass (or more), or is tuned down at least half a step, at least on the E string. I may be wrong, since I'm not great at hearing difference in octaves on the low end. Mind you, I said *octaves*, not notes. ;)

There is also not a Bb bass note on the Eb. Please keep in mind that while that may sound good exclusively on guitar, it's also telling the bass player to play a Bb, which is wrong. Crank up the bass end on the song and listen. On the chorus where it goes "You alone are worthy of my praise," the bass goes right up the Eb diatonic scale (Eb - F - G), no question about it.

On the verse where it goes from Am to D, there is an F# bass note on the D chord. So it should be D/F#.
